Immunization Clinics

Crawford County Public Health Immunization Clinics

Appointments are required  Please call 712-263-3303 to schedule all appointments

Immunization Clinics are held at:

Crawford County Home Health, Hospice, and Public Health- 105 North Main, Denison, IA

One parent or guardian must accompany the child to the clinic, if someone else will be bringing the child to the clinic; a permission slip from the parent is required. Please bring any previous immunization records to the agency at 105 N Main prior to your appointment.  We can only vaccinate children through 18 years of age. 

Please wear a mask and only bring child/children that will be getting vaccinated.

 There is a $10.00 per child charge to help cover administration costs unless your child has Medicaid. If you have Hawk-I or health insurance that pays for immunizations, you MUST go to your primary health care provider. Please bring child’s insurance card to each clinic- including if they have a Medicaid card.

Las Clínicas de Vacunas se llevan a cabo en:

Crawford County Home Health, Hospice, and Public Health- 105 North Main, Denison, IA

 Uno de los padres o guardián necesita estar con el/la niño/a el día de clínica. Si otra persona estará trayendo al niño(a) a la clínica, una nota de permiso es requerida por parte de uno de los padres. Previamente a su cita, necesita traer todo registro de vacunas previas a la agencia 105 N Main. Solo podemos vacunar a niños hasta los 18 años de edad.

Por favor de traer cubre bocas y solo traer al niño(s) que estarán recibiendo vacunas.

Hay un costo de $10.00 por niño para ayudar a cubrir el costo de administración al menos que el niño/a tenga cobertura del Medicaid. Si el niño/a tiene Hawki u otra aseguranza privada que cubre las vacunas, usted DEBE acudir con su doctor familiar para las vacunas. Por favor traiga la tarjeta de aseguranza de su hijo a cada clínica-incluyendo si tienen tarjeta de Medicaid.
