Immunization Clinics

Crawford County Child & Adolescent Immunization Clinic

Appointments are required.  Call 712-263-3303 to schedule.

We can only provide vaccines for children with Iowa Medicaid or who have no health insurance. If you have insurance, including Hawki, you must get your shots at your medical provider’s office.

Clinic is held at Crawford County Community Health—105 North Main Street in Denison.

A few things to help clinic run smoothly:

• One parent or guardian must accompany the child to the clinic. If someone else is bringing the child, a permission slip from the parent is required.
• Bring the child’s/children’s Medicaid card or other insurance card(s) to your appointment.
• Bring previous shot records to the office prior to your appointment.
• Please only bring the child or children that will be getting shots. Our space is limited.

 There is a $10.00 per child charge to help cover administration costs unless your child has Medicaid.

Hay un cargo de $10.00 por niño para ayudar a cubrir los costos de administración a menos que su hijo tenga Medicaid.
